Birth Registrations, Marriage & Death Certificates Services Available - Ministry of Government Services Website
You might be surprised by what's now available for the public through www.ServiceOntario.ca. A suite of new online services, including Newborn Registration Service and online applications, is a proud result of Service Ontario's effort to provide seamless, simple accessible and accountable services.
Register a baby using the Newborn Registration Service: This new 3-in-1 online service helps parents reduce the number of errors made on their newborn baby's registration form and allows them to quickly and easily apply for a birth certificate and Social Insurance Number – all at the same time.
Apply for Birth, Marriage and Death Certificates Online: Clients can simply fill out an online application, pay with a credit card and the birth, marriage or death certificate is guaranteed within 15 days (delivery time is included for birth certificates).
Additional Online Services: Clients can use the Online Status Checking service to quickly reference the status of recent birth, death or marriage certificate requests. Clients can also use the Change of Name Wizard to find helpful information about the ways to change a name, print forms for the two most common types of name changes, and complete them at their convenience..
For municipalities where the Newborn Registration Service has been implemented
For children born on or after Monday, December 10, 2007 at 12 am, parents may now complete the Statement of Live Birth on-line and electronically submit it directly to ServiceOntario's Office of the Registrar General. Births occurring in the Town of Halton Hills will no longer be registered with the municipality. Instead, parents are encouraged to use the Newborn Registration Service at www.ontario.ca/en/services_for_residents/STEL01_105212.html. This new service allows parents to electronically submit the Statement of Live Birth and submit applications for a birth certificate and social insurance number at the same time.
Travel letters are no longer available for births occurring on or after Monday, December 10, 2007 at 12 am. If parents are required to travel with their newborn and have not applied for a birth certificate a the time of registration, please contact the Office of the Registrar General at 1-800-461-2156 or visit www.serviceontario.ca.
To obtain an Ontario Birth Certificate:
- Obtain an application form from the Province of Ontario's website, or from the Clerk's Office.
- Send the completed application along with the fee to:
